Responsibilities

RESPONSIBILITIES:

1.1 Conduct initial intake assessments in conjunction with the Intake Coordinator &/or Clinical Coordinator..

1.2 Conduct psychosocial assessments of patients within 72 hours of admission.

1.3 Perform utilization reviews and case management as needed.

1.4 Conduct family assessments and individual therapy sessions.

1.5 Formulate and implement patient disposition from the medical center in partnership with the family, community organizations, and the multidisciplinary treatment team.

1.6 Act as a primary liaison between client, family and other staff.

1.7 Attends and participates in unit rounds. Disseminate relevant information to a multidisciplinary team. Assist in development of and review of appropriate treatment plans for individual patients by identifying actual or potential problems amenable to social services intervention and developing specific goals or interventions. Educates patients and family about disposition and discharge planning needs. Develops and implements appropriate disposition plans and communicates with treatment team, physicians, and appropriate others on these as well as with significant others as appropriate.

1.8 Participates in ongoing staff meetings, in services, and training. Participates in opportunities for staff development and improvement. Assumes responsibility for information disseminated at in services, staff meetings, and in the Communication Book.

1.9 Works in collaboration with all members of the healthcare team in providing quality patient care. Serve as a liaison to the community. Communicates information to the appropriate members of the health team and appropriate community agencies.

1.10 Works closely with the Manager to promote high degree of quality patient care while remaining fiscally efficient.

1.11 Demonstrates understanding of and compliance with hospital policies and procedures and follows regulatory agency rules.

1.12 Provide clinical leadership and supervision to social services, social work interns, and nursing staff. Promotes and maintains professionalism at all times.

1.13 Promotes customer service and client satisfaction. Advocates for patients to payors and multidisciplinary teams. Ensures that the patient's rights are upheld.

1.14 Documents clearly and legibly all findings in the clinical record and kardex as appropriate.

1.15 Performs all other duties as assigned within scope of practice.

Qualifications

MINIMUM REQUIREMENTS:

  • Minimum of 1-2 years combined experience in healthcare industry

  • Master's degree required in Social Work, Counseling, Psychology, Family Therapy, or related fields

  • Must be licensed by the Board of Behavioral Health as a LCSW, LPC, or LMFT

Pay Range

$42.82 - $71.98 / hour
