We are hiring a FULL-TIME(35 hours per week)KINSHIP CASE MANAGER for our Social Care Department with Catholic Charities for our offices serving Cattaraugus County.

    JOB SUMMARY : The role of the Kinship case manager is to provide individual and group family support through education, mentoring, advocacy and linkage services to individuals and/or their families. The Care Coordinator is a flexible, self-motivated individual seeking to ensure that all families have what they need to provide care for their families or relatives.

    ESSENTIAL FUNCTIONS :

  • Exemplify Catholic Charities' mission, vision, guiding principles and policies and procedures

  • Promote and demonstrate integrity and ethical performance, consistent with the Catholic Charities' Corporate Compliance Program, Code of Ethics, legal mandates & labor laws, managing contractual/funder regulations and accreditation standards, regulations and policies and procedures (e.g. COA, OMH, OASAS).

  • Has a strong understanding of TANF requirements, Article 6 custody and informal custody arrangements

  • Identifying and determining eligibility of community resources that families would benefit from.

  • Orients new clients to TANF applications, rules, regulations, and benefits. Assists families in applying for the TANF grant and attending the intake appointments at DSS

  • Completes a thorough initial assessment with each family to determine appropriateness for case management services. Makes appropriate referral if client does not meet admission criteria.

  • Completes required documentation within 48 hours of contact.

  • Completes initial and ongoing treatment plans within the required time frames that include the current needs, strengths, safety, and corresponding plan of care for each family

  • Provides case management services to families to include ongoing assessments, home visits, referrals, support groups and individual support as needed.

  • Utilizes an effective manner and therapeutic style that demonstrates caring, understanding, and empathy toward clients.

  • Makes active attempts to re-engage clients who withdraw from groups/support when appropriate.

  • Keeps up to date on chart requirements as well as completes case record self-audits on each client's chart as required.

  • Promptly and actively communicates with all members of client's family and designated collaterals and service provider

  • Completes all reports, 2 weeks in advance of due date.

  • Identifies pattern of multiple family needs to create curriculum to address such needs.

  • Provides outreach, advocacy, mentoring and recovery support services to those seeking or sustaining recovery.

  • Assist in the promotion of the Program through information, education, and relationship building about its services to the Communities we serve.

  • Provide direct outreach and relationship building to courts, department of community services, SPOA committee, department of social services, schools, child protective services and various other community groups with direct referral back to the Agency and its Program(s) if necessary.

  • Represent the Program and build relationships at various community events, such as school open houses, community open houses, Fairs, Forums, etc. as possible.

  • Assist in the creation of flyers and promotion information with and under the direction of Kinship Care Coordinator & District Director

  • Facilitates groups to include family events, educational groups, support groups, and outreach activities

  • Knows and understand the importance of the relationship between the agency' s in service delivery and the community.

  • Performs other duties as assigned.

  • Comply with the Agency's Compliance Program, Code of Ethics, laws, regulations, and ethical standards applicable to your job duties.

    EDUCATION : Bachelor's degree in Social Work or related field or completion of 1 year of graduate training from an accredited school of social work.

    SPECIAL REQUIREMENTS :

  • Requires travel - must possess valid NYS driver license and agency-required level of auto insurance coverage and be able to travel as needed within the eight county WNY region.

  • Must be prepared to work a flexible schedule in order to participate in position-related activities/events that may take place outside of typical business hours including nights and weekends.

    EXPERIENCE :

  • Two years of experience in the community working with families. Evidence of exceptional professional, self-motivated, interpersonal and communication (oral and written) skills required.

  • Demonstrated effectiveness with engaging with cross-functional teams

  • In-depth familiarity with WNY Cattaraugus County community and continuum of human services (health, behavioral health and social services) systems, organizations and resources

  • Fluent in various computer applications (e.g. Word, Excel, Access), electronic database systems and electronic health records; experience with graphic reporting applications preferred
